East Ann Arbor General Medicine completes successful Patient–Centered Medical Home program survey!

Congratulations to the East Ann Arbor General Medicine team, which recently completed a successful survey visit with Blue Cross and Blue Shield of Michigan (BCBSM) for the Patient–Centered Medical Home (PCMH) program! 

This program recognizes and supports Primary Care and Specialist Clinic teams that have made substantial progress implementing and using patient-centered clinical and operational processes and tools, resulting in the delivery of more coordinated, efficient and effective health care.

Each year, BCBSM randomly selects 1-2 Michigan Medicine primary care (Medical Home) and 1-2 specialty (Neighborhood) sites to participate in this survey process. The sites spend significant time preparing for these visits. The PCMH philosophy is built on 13 initiatives. Each initiative focuses on a domain of function and a set of defined capabilities/standards that will enable the site to achieve the vision for that domain of function. The sites are required to verify/demonstrate how these capabilities are operating.

The overarching goal of the program and survey process is to strengthen the role of primary care and specialist physicians in the delivery and coordination of care to build an integrated, well-coordinated medical neighborhood.
