Revenue Cycle Mid-Serve, formerly Health Information Management, set to move locations

Revenue Cycle Mid-Service, formerly Health Information Management, will be moving to 700 KMS Place in Ann Arbor on Monday, Oct. 5.

Starting that day, please direct all documents to be imaged into the patient’s medical record and all patient medical record requests to KMS (700 KMS Place, Ann Arbor, MI 48108).

All inter-office mail, loose sheets and medical record imaging requests must be sent to KMS effective on the day of the move.

Please note, all patient walk-in requests for medical records have been suspended until further notice due to COVID-19. Patients may continue to receive records via the patient portal, phone (734-936-5490) or by mail.
