How to add self-health screening tool to your personal device home page

The easiest way to access the self-health screening tool regularly is to make it a bookmark or icon on your smartphone. To do that, follow the directions below, depending on your device:

iPhone or iPad:

  • Open the website (https://healthscreen.umich.edu) in your Safari browser.
  • Tap the “Share” icon at the bottom of the page and select “Add to Home Screen”:
  • You will be able to name the icon whatever you would like. It will then appear on your phone’s home screen.
  • Click the icon whenever you are ready to enter a Michigan Medicine facility and fill out the brief questionnaire.
  • By using this tool and displaying the green check box on your phone to gain access, facility access will be faster and safer than waiting in crowded lines.

Android device:

  • Open the website (https://healthscreen.umich.edu) in a browser.
  • Tap the menu button and tap “Add to homescreen.” You’ll be able to enter a name for the shortcut and then the browser will add it to your home screen:
  • You will be able to name the icon whatever you would like. It will then appear on your phone’s home screen.
  • Click the icon whenever you are ready to enter a Michigan Medicine facility and fill out the brief questionnaire.
  • By using this tool and displaying the green check box on your phone to gain access, facility access will be faster and safer than waiting in crowded lines.
