Health care providers asked to participate in brief sleep and wellness survey

The Sleep and Circadian Research Laboratory in the Department of Psychiatry is conducting a study to learn more about the effects of the COVID-19 stay-at-home policies on the sleep, health and working behaviors in health care providers.

To gather more information, the team would like caregivers to complete a brief survey. IT should take about five minutes to complete the survey.

Please note, answering this survey is voluntary. You don’t have to answer it if you’d rather not. It is possible that some of the questions may make you feel uncomfortable. If a question makes you uncomfortable, you can just skip it and go to the next question. To keep your information confidential, the laboratory is not collecting any individually identifiable information of the participants.
