A streamlined system for medical records now provides seamless patient care and improved collaboration between Michigan Medicine and its partner, Trinity Health.

Trinity, a national health system, recently adopted Epic, the electronic health record system that supports MiChart. The first phase of implementation in January 2020 included facilities which serve patients in southeast and west Michigan.

Previously, providers had to fax medical records manually, or use Great Lakes Health Connect, an intermediate site which was difficult to use and had incomplete information.

Now providers can click a tab (Care Everywhere) in MiChart to access records from Trinity’s epic “TogetherCare.” This one click connection will ease the transition of roughly 180,000 patients annually who seek care at both health systems simultaneously.

Trinity has partnered with Michigan Medicine since the late 1970s. Trinity/Michigan Medicine partnerships in Southeast Michigan include St. Joseph Mercy Ann Arbor and St. Joseph Mercy Chelsea. In west Michigan, the partnership involves Mercy Health Muskegon and Mercy Health Saint Mary’s.
