Michigan Medicine conducts and publishes Community Health Needs Assessment (CHNA) and Implementation Plan (CHNA-IP) every three years.
Many of the organization’s local initiatives, such as the Ypsilanti Meals on Wheels (YMOW) program or the Medication Assisted Treatment (MAT) Program, are based on information gained from such assessments.
These are good examples of a systematic approach to addressing health disparities in the community. More about 26 current projects outlining our community efforts can be found here.
