Learn about internal programs and resources at upcoming fair
Michigan Medicine has many ambulatory and specialty care programs and resources available to patients, and it can be difficult to know about them all!
These programs are doing great work for patients and families every day. On Tuesday, Dec. 3, nearly 50 different programs will be available to share information with faculty and staff about their unique services and resources.
They will come together as part of the inaugural Internal Program and Resource Fair at Michigan Medicine. The event has been organized by the Care Team Coordination Group, which is made up of representatives from both ambulatory and inpatient staff including the Complex Care Management Program, Inpatient Nurse Case Managers, Nurse Care Navigators and the Inpatient and Outpatient Social Work departments.
The overarching goal of the group has always been to improve communication across the care continuum. This group wants to ensure that faculty and staff are aware of these amazing resources and that they know how to connect patients and families to them!
