Tell us how your unit or department advances community health
Use the Community Benefit reporting form to submit your activities by June 20
This April, UMHS faculty and staff began submitting Community Benefit reports for activities and efforts their departments or work units provided during fiscal year 2014 to support our community.
Called “Community Benefit,” these activities help increase access to care and/or improve community health. Please submit your reports before June 20.
Tell your Community Benefit story with the downloadable Community Benefit reporting form, and email it to Keven Mosley-Koehler, coordinator of Community Benefit and Community Health Assessment at email@example.com. Submit reports for activities held during fiscal year 2014 (July 1, 2013 – June 30, 2014).
Community benefit activities assist the poor, uninsured and underinsured and are offered free or at a low cost. These activities exemplify our health system’s commitment to community health while serving to maintain our non-profit tax-exempt status.
Your submissions help populate an annual, health system-wide Community Benefit report for the Michigan Health and Hospital Association Community Benefit survey which describes hospital community-focused efforts across Michigan and provides an opportunity for UMHS to assess its investment in advancing community health.
Reportable activities are offered to the community at large, for free or at a low cost, and include many topics and approaches. Click here to see some examples.
